Serious Incident Notification and Review Process

This document details the processes that are in place within the St Helens Safeguarding Children Partnership (SHSCP) in relation to Serious Incident Notifications, notification of the death of a care leaver, Lower-Level Case Reviews, Rapid Reviews and Local Child Safeguarding Practice Reviews (LCSPR).

St Helens Safeguarding Children Partnership Serious Incident Notification and Review Process

The below form should be completed to notify the Safeguarding Children Partnership of:  

  • Serious Incident 
  • Death of a Care Leaver (up to & including the age of 24) there is no necessity for abuse or neglect to be present when making this notification. 

&/or 

  • Request a Rapid Review / Local Child Safeguarding Practice Review
  • Request a Lower-Level Review / Good Practice Review
