Enhancing Families Programme - pilot to start 1st December 2021


The Family Nurse Partnership (FNP) Service is ending and being replaced with an alternative pathway for vulnerable families in St Helens. The reason we are changing the approach is to enable a wider group of families to access needs led additional support, this will be called Enhancing Families Programme.

A six-month pilot programme will commence on the 1st of December 2021. This will be an iterative process to test the criteria and capacity and will be reviewed monthly.

Partners will refer into the service via a referral form which will be distributed to partner agencies prior to the launch date and will also be available via the dedicated children and young people’s website provided by Wirral Community Health and Care NHS Foundation Trust www.wchc.nhs.uk/EFPThe route into the service is via wchc.enhancingfamilies@nhs.net on the referral form. Referrals will be screened at weekly team meetings with the Specialist Nurses and Service Lead.
